Clinical Director – Dr Jonathan Andrews
I am the Clinical Director for Eastbourne East Primary Care Network (PCN) and the senior GP partner at Princes Park Health Centre. I have been a GP since 1990 and have worked in the Eastbourne area since 1994.
In my role as Clinical Director, I am responsible for supporting collaborative working across our member practices, fostering strong relationships to enhance the delivery of primary care. Our focus is on improving access and outcomes by providing proactive, personalised, and integrated healthcare.
A key priority for our PCN is to develop and deliver non-traditional, enhanced access to services. By building a varied and multidisciplinary workforce, we aim to meet the evolving needs of our patients and the wider community through proactive, community-centred care.

I have worked in Primary Care for 27 years and joined Eastbourne East Primary Care Network (PCN) in March 2022. As PCN Manager, my role is to support the Clinical Director in ensuring the smooth and effective operation of the network.
Together, we oversee the delivery of contractual requirements set by the Integrated Care Board (ICB) and NHS England, with the aim of delivering high-quality care and reducing health inequalities across our PCN population. We work closely with our member practices and community partners to ensure a collaborative approach to improving outcomes for our patients.

All 5 of our pharmacists carry out the following:-
Medication reviews including structured medication reviews, DOAC and asthma reviews.
Discharge and hospital medication changes as requested by the hospital consultant.
Patient queries
They also undertake any surgery projects to ensure safe prescribing and continued medication monitoring.
Community Paramedic
PCN Community Paramedic – Kate
Hello, my name is Kate and I have been a paramedic for 15 years. I began my career with London Ambulance Service where worked on a response car, ambulances, the Hazardous Area Response Team and mentoring new Paramedics. In Surrey I worked in Primary Care and I have recently relocated to the East Sussex area joining Eastbourne East Primary Care Network (PCN) as their Community Paramedic. My main focus here will be to support the PCN frailty and housebound patients.
Out of work I love to travel with my dog by my side. My interest in health and wellbeing extends into my time off, as I teach yoga and new yoga teachers.
Nurse Practitioner
PCN Nurse Practitioner – Julia
I am a Nurse Practitioner working across all three surgeries within our Primary Care Network (PCN), providing comprehensive care and clinical support. I have a special interest in Dementia and Frailty, and lead dedicated Dementia Clinics as well as regular ward rounds for care home residents across the PCN. My role involves proactive management of complex patients, supporting early diagnosis and holistic care planning to improve outcomes and quality of life for some of our most vulnerable populations.
Care Coordinator Team
Leesa – Princes Park Health Centre
I have been a Care Coordinator with Eastbourne East PCN since October 2023, working at Princes Park Health Centre. In my role, I support care home residents, dementia patients, and their families or carers by assisting them in accessing the services they need.
I work closely with our Nurse Practitioner, who specialises in elderly and frailty care, to ensure coordinated and compassionate support for our patients. Additionally, I manage the Digital Ambassador service, helping patients resolve any issues they may encounter with the NHS App, promoting easier access to digital healthcare tools.

Chris – Harbour Medical Practice
I have been working as Harbour’s Care Coordinator since June 2021. In this role, I support local care home residents, palliative patients, and those who are elderly or frail—helping to ensure they receive appropriate and timely care.
I also accept referrals from the clinical team for patients who may benefit from additional support within a social or community-based setting, aiming to improve their overall wellbeing and quality of life through personalised care coordination.
Rhiannon – Stone Cross Surgery
I work at Stone Cross Surgery and joined the team in October 2023. In my role, I work closely with the PCN Social Prescriber to support patients with cancer and those receiving palliative care, ensuring they have access to the services and support they need.
I also assist patients with learning disabilities & diabetes, helping them navigate healthcare services and supporting them through their annual health reviews. In addition, I accept referrals from the clinical team for patients who may benefit from extra support and guidance, offering a personalised approach to care and wellbeing.
Dawn – Stone Cross Surgery & Princes Park
In my role, I support patients across the network by helping to coordinate their care and ensure they receive the right support at the right time.
I also assist in managing the appointment schedules for our Pharmacy team, helping to keep their clinics well-booked and running efficiently. In addition, I support Stone Cross Surgery with the management of their Mental Health register—ensuring that patients are well cared for before, during, and after their annual reviews, with a focus on continuity and personalised support.

I have been the Social Prescriber for Eastbourne East PCN since September 2023. My role is to support patients in improving their overall wellbeing by focusing on their individual needs and circumstances.
I provide a wide range of support, with a particular focus on helping those who are experiencing loneliness or have difficulty accessing social activities and community resources. With many years of experience in the care sector, I am passionate about empowering people to take control of their lives and make positive changes to support a healthier, happier lifestyle.

Hello, my name is Vicki Cowen, and I am the Health and Wellbeing Coach for Harbour Medical Practice, Princes Park Health Centre, and Stone Cross Surgery.
I have worked in the wellbeing industry for over 20 years, with a background as a sports and remedial massage therapist and Pilates teacher. I joined Eastbourne East Primary Care Network (PCN) as their Health and Wellbeing Coach in October 2022.
I am passionate about all aspects of healthy living and take great pride in supporting patients to build confidence and take an active role in managing their own health and wellbeing. Coaching individuals to make sustainable, positive lifestyle changes is at the heart of what I do

First Contact Practitioner (FCP’s)
FCPs are experienced, senior physiotherapists with advanced training in the assessment and management of musculoskeletal (MSK) conditions.
Appointments can be booked directly through reception at any time during surgery opening hours—there’s no need to speak to a GP first.

I offer face-to-face appointments to help assess, diagnose, and advise on a wide range of MSK issues, including:
- Soft tissue injuries, sprains, strains, or sports-related injuries
- Arthritis affecting any joint
- Muscle, ligament, tendon, or bone problems such as tennis elbow, carpal tunnel syndrome, or ankle sprains
- Spinal pain, including lower back, mid-back, and neck pain
- Spinal-related pain in the arms or legs, including nerve symptoms such as pins and needles or numbness
I can also request X-rays or blood tests if needed, without the need for a GP referral.
Please note: I only see patients aged 16 and over
Jessie – First Contact Practitioner (FCP), working for Stone Cross Surgery.

As an FCP, I am specially trained to provide expert assessment, diagnosis, and management advice for musculoskeletal (MSK) conditions directly within primary care. Patients can book an appointment with me without needing to see a GP first.
I bring experience from both the NHS and private sector, having worked as a specialist Physiotherapist in a range of settings. When necessary, I can request investigations such as X-rays or blood tests and make onward referrals to other services for the continued management of MSK conditions.
My aim is to support patients in accessing the right care quickly and effectively, helping them to manage and recover from MSK issues with confidence.

Mental Health Support Coordinator

I am part of the Emotional Wellbeing Service and joined Eastbourne East PCN in June 2024.
My role focuses on identifying patients on the Serious Mental Illness (SMI) register who may benefit from additional support in attending their annual physical health checks. I work to engage with these patients in a supportive and approachable way, helping to reduce barriers to care.
Where appropriate, I can carry out the physical health checks myself, helping to ease the demand on GP surgeries. I also make referrals to other services as needed, ensuring individuals receive the right support for both their physical and mental wellbeing
Mental Health and Wellbeing Advisor
Georgie – Screen & Intervene
As part of the Emotional Wellbeing Service, I work as a Health and Wellbeing Advisor providing non-clinical, holistic support to patients on the Severe Mental Illness (SMI) register following their annual physical health check.
I help patients to understand and explore the outcomes of their health checks, support them in creating personalised health improvement plans, and connect them with relevant services, community groups, and activities. My goal is to empower patients to improve how they feel about their health and overall wellbeing
